Health Office
District 6 Health Services encourages every child to be immunized in consultation with their health care provider. Vaccinations prevent many serious diseases and help prevent the transmission of those contagions in our schools, which improves attendance and learning for all students. Colorado law requires students who attend a public, private, or parochial kindergarten-12th grade school to be vaccinated against many of the diseases vaccines can prevent unless a certificate of exemption is filed. If your student is ill and you aren't sure if you should send them to school, HERE is information about when to keep your student home for illness.
Our health clerk is Rose Downs, and our school nurse consultant is Kianna Perez, RN. The health office is open 7:30 am-4:00 pm Monday through Friday. During this time, Rose is in the health office. Kianna is in the building at least once a week.
The health office serves multiple functions. We see students and staff for illness or injury during the school day, administer prescribed medications, and maintain medical and immunization records. Additionally, we can serve as an interface between health care providers and teachers. If your student has health issues, let us know, we can help!
If your student needs medication or treatment while at school, we can help. We need a completed medication authorization form from an approved health care provider (physician, nurse practitioner, or physician assistant) with a parent's signature. Bring the authorization and medication (in pharmacy-labeled packaging) to the health office to get started. Provider authorization forms can be found below or on the District 6 Health Services page under STUDENT HEALTH CONCERN INFORMATION AND FORMS.
